Your First Visit

As a courtesy to our patients, we have streamlined the registration process by providing links below to the forms required for your first appointment at In His Hands Family Medicine.  By simply clicking and printing the documents you will then be able to review and complete them in the comfort of your own home prior to your visit. 

If you are a new patient, we will not prescribe controlled medications, for example pain medications like hydrocodone, oxycodone or tramadol or anti-anxiety medications like Xanax, Klonopin or Valium, at your first visit.  We do not prescribe weight loss medications, like phenteramine, Qsymia or Sanorex at all, or stimulant ADHD medications for adults, like Adderall, Concerta or Vyvanse.  Please do not come into our office with the expectation that we will prescribe these particular medications for you.
